Roots Academy Canada Reimbursement Form

This form is to be used by employees and volunteers requesting reimbursements from Roots Academy Canada for academy-related or event expenses.

Receipts, invoices, and other relevant documentation must be uploaded below for the reimbursement to be processed. Original receipts are required - All receipts must be itemized and show proof of payment.

(Credit card receipts are not itemized, but they show proof of payment.)‍
